Adult Day Health Mission Statement
Saint Francis Adult Day Health Program strives to provide compassionate, loving care rooted in dignity, reverence and respect for life to the Participants of the program. The program is committed to enriching the lives of elders, disabled adults, their families, caregivers and the community through specialized adult day services, respite care, education and support.
Program Philosophy & Purpose
Saint Francis Adult Day Health Program is dedicated to providing a service, which enables elders and impaired adults to remain in the community, for as long as feasible. The purpose of Saint Francis Adult Day Health Program shall be:
To provide daytime care for adults who are functionally, emotionally, and/or cognitively impaired (including Alzheimer’s Disease, or related dementia, Parkinson’s Disease, Stroke, Heart Disease, Macular Degeneration/Blindness and other impairments.)
To provide respite for families caring for adults.
To provide family support and supportive counseling to families with adult persons in their care.
To provide referrals to community resources and educational opportunities for caregivers through agencies offering caregiver support for in home care management of their loved one (including Alzheimer’s /dementia care).
To provide a center where students may receive training in working with seniors.
To delay premature institutionalization.
Our Adult Day Health Program provides a safe, supportive environment for frail or functionally impaired older adults who may otherwise not be able to be left safely at home alone. Additionally, we provide support, respite and education to caregivers (including education on Alzheimer’s care and/or related dementia). The program is designed to promote and enhance the social, physical and cognitive skills of participants. To gain meaning from our program participants are offered opportunities to:
Develop relationships and interact with others
Discover new interests
Improve and/or maintain physical skills
Enhance cognitive skills
Adult Day Health Care Services
Each and every individual who enters the plan is treated like a member of our family. Our nurses work with the individual's private physician to develop a personalized treatment plan, ensuring that we give the best possible care and attention to the individual's special needs. Saint Francis Adult Day Health offers Physical, Occupational and Speech Therapy during Program hours as insurance allows.
Our comprehensive program encourages participants to engage in a variety of activities that provide both physical and mental stimulation, such as:
- Intergenerational programs
- Special Events
- Craft Programs
- Daily Mass
- Pet Therapy
Each of our program participants has their own strengths and care needs. Our nurses work with each individual's primary care physician to develop an individualized treatment plan. Services include:
- Medication education, monitoring, and administration, including injections as needed
- Coordination of physical, occupational and speech therapies available
- Personal care
- Health screening and cardiopulmonary assessment
- Health education
- Nursing treatments
- Nutrition services
- Podiatry, eye, dental care and audiology available on a fee for service basis
- Collaboration with hospice services
We also sponsor a monthly Caregiver Support Group. The group is open to the public and meets on the fourth Monday of every month at 5:00pm at the Adult Day Health center, 37 Thorne Street, Worcester MA. Please see our Events page for scheduled dates.
